This is a one time, 90 minutes session with a MC360 Certified Health Coach. 

Cost and Payment:  

• This appointment is $499.
• Our services are not covered by insurance 
• We are unable to submit to insurance or provide superbills 

Appointment Rescheduling: 

• 1 free reschedule per appointment with 3+ business days’ notice 
• Cancellations or reschedules within 3 business days incur a $200 fee 

Getting Started – What Happens Next: After payment, you’ll be onboarded to our client portal where you’ll receive 2 required forms: 

• Intake Form – helps us understand your health history 
• Office Policies and Client Agreement – outlines how we work together 

Both forms must be filled out and returned 5 days before your appointment. To ensure we can provide the best possible care, we reserve the right to cancel and reschedule your initial appointment if forms are not complete.
