Welcome! This brief pre-screening will help us understand your health needs and share key info about our whole-person approach so we can ensure we’re the right support for you.

Here’s how it works:

1. Start with the Pre-Screening
   Click “Start” and answer a few quick questions based on your current health.
2. Receive Your Invitation to Apply
   Once you know you are a fit, you’ll be guided to complete the full application.
3. Complete the Full Application
   Set aside about 30 to 60 minutes to complete your application in one sitting. You’ll be asked about your current health and history. Focus on key concerns and your big-picture health story – no need to include every detail. Once you’re approved and in the client portal, you’ll be able to upload any labs, doctor’s notes, or other documents.
4. Submit and Watch Your Email
   You’ll get a confirmation email once your application is submitted.
5. Next Steps
   We know you are eager for help and we review applications as quickly as possible, often within just a few weeks. Once accepted, our team will reach out. You’ll receive your next steps including a few quick forms to sign. Then you can schedule your first appointment!
